BUKIT MERTAJAM: A foreign worker was killed after he was crushed by a forklift last Monday.

The incident happened in a manufacturing factory here.

Following the incident, an investigating team from the Department of Occupational Safety And Health (DOSH) has carried out an investigation. 

"Initial investigations showed the victim was a forklift driver in the factory.

"Prior to the incident, the victim was moving and transferring items in the logistics area of the factory.“

"He was reportedly crushed by the forklift during the transferring process. He died at the scene," the department said.

DOSH said an immediate stop work order was issued pending the completion of investigation.

"We have also ordered the employer to conduct an internal probe to ascertain the cause of the incident and identify improvement measures.

"The employer will have to present their findings to us," it further added.

DOSH Penang will conduct further investigations by getting relevant documents and recording statements from witnesses.

It will also re-visit the factory after improvement measures have been taken.

 "It is the responsibility of the employer under Section 15(1) of the OSH Act 1994 to ensure the safety at workplace.

"We view seriously every occupational safety and health issue at workplace in all sectors," it said.
